Mine Safety and Health Administration has released a final report about an accident that took the life of a miner

Oak Hill, WV (WOAY) – The Mine Safety and Health Administration has released a final report about an accident at Panther Eagle Mine that took the life of Aaron Warrix in April.

According to the report, Warrix was working to install turn posts when a five-foot-long and three-foot-wide piece of rock fell on him.

He was taken out of the mine and airlifted to Charleston Area Medical Center, where he was pronounced deceased.

Investigators say the accident was caused by an improperly supported roof. Supervisors also failed to identify the danger before it was too late.
